The I-SNP Model is Reimagining Care for Nursing Home Residents

Two recent reports offer important validation of what we see every day at TruHealth: when healthcare is designed around the unique needs of nursing home residents, the results can be meaningful—for residents, families and care teams.

TruHealth is the clinical services arm of American Health Plans, the nation’s leading provider-owned I-SNP. Together, we partner with more than 50 nursing homes and serve nearly 10,000 members across 14 states, bringing coordinated, value-based care directly to residents where they live.

New analysis from Bentley Health Strategies shows I-SNP enrollment has grown by roughly 40,000 members since 2021, reaching nearly 130,000 nationally. At the same time, research published in Geriatric Nursing highlights the value of an in-house nurse practitioner model, emphasizing proactive decision-making, early identification of health changes and close collaboration with residents, families and facility staff.

For those of us delivering care through the I-SNP model, these findings help validate what we experience every day: care works better when the health plan and care provider are aligned and the clinical team is present at the bedside.
