Model of Care
Our Model of Care brings together clinical and care management team members to manage the care of the ISNP member. In addition to their physical needs, American Health Plans also offers Member Advocates who manage the mental and emotional needs of the members and help ensure they take full advantage of the benefits included in their plan.
